LPT Membership Agreement

This membership agreement between the member signed below and Legacy Personal Training can be terminated at any given time. A charge of $25 will be made for any checks returned due to insufficient funds. Late payment and payments with insufficient funds violate the gym rules. Membership cancellation fee of $250.

    One time enrollment fee of $80.00

    Open gym (no group classes)

    $40/month with a 12 month commitment$60 month to month

    Group classes (including open gym)

    $80/month with a 12 month commitment$120 month to month$110 for a punch card (10 uses per card)

    Jiu Jitsu:

    $80/month with a 12 month commitment$120 month to month

    Unlimited gym access (during gym hours) and family packages available. Please contact staff for pricing.

    Members agree to keep an obey all the rules and regulations of Legacy Personal Training. By signing below I agree that I have received, read and understand my right regarding cancellation and payment options and agree to the terms of this contract.
